Position Summary: The Behavioral Health Coordinator plays a critical role in the health care team- specifically in clinics and departments specializing in behavioral health treatment, or with high volumes of behavioral health clients. The position ensures comprehensive and thoughtful connection to clinical and non-clinical supports for children and families to ensure efficient and thoughtful discharge from outpatient services and to enhance stability in the community.

Essential Functions:

The position offers to provide care coordination services for children and youth with special health care needs or at risk and vulnerable from the social determinants of health by linkage with a medical home, community resources, funding agencies, educational support and behavioral health supports.

The Behavioral Health Care Coordinator uses a lens of behavioral health understanding and experience to meet the specialized needs of children and youth struggling with mental health.

The position requires a calm presence and an ability to manage and support high stress, at risk youth and families struggling with behavioral health..

General Duties:

• The position offers to provide care coordination services for behavioral health support.
• Assist with center and organization in achieving optimal quality, financial and performance metrics.
• Monitor providers’ schedules to ensure all patients are scheduled appropriately and in a timely manner.
• Assist with Patient Engagement initiatives and actively work to improve the patient experience.
• Serve as liaison between clinical, administrative staff and other ancillary services.
• Work with front-end team to reduce first pass denials, secures eligibility, referrals, and capture of point of service cash.
• Assist with daily cash collection through reconciliation and cash control policy enforcement.
• Responsible for maintaining a pristine internal and external environment; report all facility and grounds issues, as appropriate.
• Report and submit incident reports, when appropriate
• Assist with behavioral health referrals and scheduling.
• The position requires a calm presence and an ability to manage and support high stress, at risk youth and families struggling with behavioral health.
• Compliance with and control of confidential data ensuring privacy and security standards
• Responsible for supervision and coordination of day-to-day administrative operations, provides supervision of assigned practices to ensure adherence to all Tampa Family Health Center (TFHC), HRSA, State, Federal and accreditation agency rules, and regulations
• Demonstrates Service Excellence and always conveys professionalism and strong motivation to achieve optimum results
• Assists internal staff, other health care providers, and third-party payers in performing QI chart audits
• Ensures a patient-centered environment that promotes excellent communication and teamwork
• Demonstrates support for the mission, values, and goals of the organization through behaviors that are consistent with the Tampa Family Health Centers
• All other duties as assigned.
