Behavorial Social Worker

University Home Care Inc is seeking a compassionate and dedicated Behavioral Social Worker to join our team in Livonia, MI. In this role, you will provide essential behavioral health services to support the well-being of our clients in a home care setting. You will work closely with clients and their families to assess needs, develop care plans, and coordinate services to enhance quality of life.

Responsibilities

• Conduct comprehensive behavioral health assessments to identify client needs and goals.


• Will be going to client home.


• Develop individualized care plans in collaboration with clients, families, and healthcare providers.


• Provide counseling and support to clients and their families to address behavioral and psychosocial challenges.


• Coordinate with interdisciplinary teams to ensure continuity of care and access to community resources.


• Advocate for clients’ rights and access to appropriate behavioral health services.


• Maintain accurate and timely documentation in compliance with agency and regulatory requirements.


• Participate in ongoing training and professional development activities.

Requirements

• Master’s degree in Social Work ( BSW) or (MSW)  from an accredited program.


• Licensed Clinical Social Worker (LCSW) or eligibility for licensure in Michigan.


• Experience in behavioral health, preferably in home care or community settings.


• Strong interpersonal and communication skills.


• Ability to work independently and as part of a multidisciplinary team.


• Knowledge of relevant state and federal regulations governing home care services.


• Valid driver’s license and reliable transportation to travel to client homes.
