Job Description - Bereavement Coordinator


The Bereavement Coordinator is responsible for planning, implementing, and evaluating bereavement services for patients' families and community. This role includes providing emotional support, developing individualized care plans, coordinating community resources, and offering grief support. The coordinator ensures that families receive compassionate support after the loss of a loved one, in alignment with hospice care values.


Requirements

Bachelor's Degree in social work, counseling or related area required, with state licensure as appropriate in Kentucky and Ohio. Prefer at least one year experience in a health care setting, with knowledge of current trends in bereavement care and end-of-life experience.  Experience in the area of grief and loss support and group activities.  Demonstrates ability to communicate professionally with clients, team members, providers, and healthcare professionals, displays personal qualities of empathy, responsibility, diplomacy, dependability and integrity.

Must demonstrate effective communication skills with team members, clients, and family members.  Must have the ability to work independent of direct on-site supervision.  Demonstrates leadership and teaching ability.  Must arrange travel and be able to access client homes independently.   Must carry state minimum state minimum in automobile liability insurance.  Full time with benefits, Monday - Friday 8:00 am to 4:30 pm, including evening hours for groups and events.
