Work Location:

Indianapolis Office 

Schedule is Full Time 40 hours per week

Job Functions:

• Provide administrative support to the Bereavement Program  
• Handles bereavement mailings tasks with the assistance of the volunteer program 
• Organize and maintains all bereavement records and spreadsheets 
• Facilitate the efficient delivery of all Bereavement services. 
• Complete all routine plotting duties and incoming plotting requests as needed and in a timely manner. 
• Complete admission bereavement plan of care task and update plan of care based on updates from the IDG team.  
• Maintains up to date knowledge of Medicare Hospice bereavement guidelines and ensures compliance. 
• Plots bereavement tasks including phone calls and visits. Ensuring plotted tasks follow POC frequencies. 
• Provides bereavement calls and support to beavered according to the level of need. 
• Phone etiquette: Demonstrate excellent customer service and communication skills, maintaining a high level of professionalism and working to establish a positive rapport with every call. 
• Refers bereaved to appropriate mental health/community support programs as appropriate. 
• Organizes and facilitates the bi-annual celebration of life programs. 

Qualifications:

• Valid state driver’s license and reliable automobile. 
• Proof of current automobile insurance. 
• Current health certificate/physical examination and TB testing. 
• Criminal background check.  
• Able to communicate effectively in English, both verbally and in writing. 
• Additional language proficiency an asset.  
• Flexible, yet organized, with ability to exercise sound judgment. 

Credentials:

• Current driver's license
• Must have reliable transportation and insurance
