Acting as a liaison in the Sunset Park community between social service agencies, mental health agencies, and families and making relevant referrals
Accompanying families to school meetings as well as appointments to access services in the community
Visiting families’ homes
Collaborating with CFL program staff, school staff, ACS, Public Assistance, Medicaid, Housing Authority, multiple systems and other community providers
Providing culturally competent preventative and foster care services, which includes collaborating with families to develop and review service plan goals and their implementation
Conducting psychosocial assessment and home assessments
Attending weekly supervision with a LCSW clinical supervisor
Leading parenting groups and workshops within the community
Managing a flexible work schedule, which includes 3 evening per week
Maintaining timely case record/documentation requirements
II.QUALIFICATIONS
Education/Degree minimum requirement: Bachelor’s Degree with 2 years of experience working with families, or Master’s Degree in Social Work (MSW) or equivalent mental health professional
Previous experience working with children and families
Highly organized and able to manage time efficiently
Flexibility and the ability to work well as a member of a team
Ability to set limits and maintain helping role of practitioner and to intervene appropriately
Bilingual: Spanish or Chinese/Mandarin, required

IV.WORKINGCONDITIONS
Share an office with one other program staff
Access to an individual desk and computer
May also attend various meetings with different personnel and supervisors
Required to travel to conduct home and community visits
V.SCOPEOFRESPONSIBILITY
The Bilingual Case Planner is responsible for the planning and management of families in preventative programs, yearly group, home, and community visits, process treatment plans, statistics, and all required documentation.
This is a full-time non-exempt position at 35 hours per week. The salary range is $45,000 to $53,000 per year.
