Schedule: This position follows a full time schedule with hours varying. Flexibility may be required based on patient and operational needs.

Essential Duties and Responsibilities:
1. Complete an Assessments tool with patients that arrived at our ED, and or Ambulatory Clinics
2. Assist with community resources and scheduling appointments
3. Facilitate referrals for clinically indicated services outside the primary care clinic (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment, etc.).
4. Refer to primary care practices for ongoing follow up and engagement with a primary care provider
5. Support and closely coordinate behavioral health care with the patient’s PCP and, when appropriate, mental health providers.
6. Encourage patients to become actively engaged in their own health through calm, compassionate communication and by utilizing techniques such as motivational interviewing and behavioral activation.
7. Accurately document patient information, assessments, interventions and encounters.
8. Attend ongoing training and learning related to job position and duties.
9. Proactively outreach to patients to encourage them become actively engaged in their own health through calm, compassionate communication and by utilizing techniques such as motivational interviewing and behavioral activation.
10. Serve as a resource for participants in the program.
11. Develop referral relationships with healthcare providers, social workers and other community based service providers.
12. Schedule patient appointments.
13. Strong organizational and planning skills.
14. Provides outreach to the community.
15. Assist in the development of resource materials and preparation and maintenance of accurate records and data.
16. Complete all reports to document activities and provide data to measure impact.
17. Performs other administrative office duties.
18. Maintains department orderliness and cleanliness.
19. Other duties as assigned.
