Job Description - Bilingual Host Homes Case Manager

TARGET START DATE: September 1, 2025



ROLE: The Bilingual Host Homes Case Manager provides a high-level of wrap-around & social service support to both English and Spanish-speaking Youth Participants and Host Families in the Host Homes Program. This role works collaboratively to prepare & sustain families to host youth experiencing housing instability, in addition to case managing youth participants with the goal of stable housing, education, employment, financial literacy, health, mental health, interpersonal relationships, and general self-sufficiency. https://watogether.org/What- We-Do/Host-Homes 



HOURS: Regular Full Time, 40 hours per week, 1.0 FTE. Occasionally scheduled, rotational evening and weekend hours may be required, as well as occasional out-of-town travel. 



COMPENSATION:  Total salary range for the position is $27.30-$33.30 per hour. Candidates hired for this position can expect a starting wage of $28.30 - $29.80 per hour, depending on qualifications and experience. LOCATION: Hybrid - TOGETHER! Headquarters Office, various Host Homes sites including but not limited to Host Family residence, secondary schools in North Thurston Public Schools, Tumwater School District, Olympia School District, home/remote office, and various points of contact with Host Families and youth within Thurston County. 



ACCOUNTABLE TO: Host Homes Program Director, Host Homes Lead Case Manager



DIRECT SERVICE DUTIES:


1. Provides specialized services to youth and young adults in education, employment/career development, housing, and other designated fields and supports them in achieving their goals.
2.    Case Manages appx. 8-10 youth through the lifecycle of the program. Uses the TOGETHER! in- take, follow up and other data tracking systems to track youth progress and keep data organized for reporting requirements.
3.    Case Manages respective Traditional & Kindship Hosts through lifecycle of the program.
4.    Builds strong relationships with a focus on mentorship and trauma-informed care to Host Homes participants to effectively provide quality services.
5.    Develops and maintains individualized, strengths-based, youth-driven service plans.
6.    Teaches life skills, job readiness or other subject matter to youth and families.
7.    Builds and maintains collaborative relationships with funding liaisons, community service providers, businesses and educational institutions related to specialized services


8.    Works with school staff, community members and partner agencies to support youth success in the program. Develop and nurture relationships with these sectors.
9.    Transport youth participants, in own personal vehicle, to Individual Service Plan- related appointments and activities in accordance with TOGETHER! Policies and Youth Program Procedures.
10.    Participates in School District student intervention teams when needed.
11.    Participates in Immigration-related advocacy when needed.
12.    Recruits and educates interested Hosts through collaboration with program leadership, district and community partners.
13.    Trains and manages Hosts to serve in the Host Homes Program effectively and
sustainably.
14.    Screens, and co-interviews Host families and prospective Host participants. Conducts home inspections, reference checks, and training for Hosts.



ADMINISTRATIVE & GENERAL DUTIES:
1.Maintains clear, professional, timely participant contact notes, service plans, progress updates, outcomes, and other documentation in designated databases.
2.Maintains data processes and credentials to access internal and external databases.
3.Tracks, monitors and reports own monthly financial expenditures related to case management, direct service and programming.
4.Coordinates and tracks check requests for Host reimbursements and youth stipends.
5.Attends bi-weekly Program Team Meetings.
6.Attends regular, ongoing trainings on topics relevant to the Host Homes Program, including supporting LGBTQ+ and Youth of Color experiencing housing instability, positive youth development, trauma informed care, and Host Home Model best practices.
7.Attends TOGETHER! agency training sessions and staff meetings.
8.Represents TOGETHER! and the Host Home Program at public events, as needed.
9.May occasionally provide supervision and training to new-hire case managers, volunteers, student-interns, and others to ensure adherence to the Host Home Program policies and procedures.



REQUIRED QUALIFICATIONS & SKILLS:
1.At least 18 years of age.
2.Bilingual in English and Spanish verbal, written and reading fluency.
3.Bachelor’s degree in Community Health, Social Work, Psychology, Social Sciences, Education,or closely related fields. This requirement may be waived for individuals with 5+ years ofintensive youth case management experience.
4.Minimum 2+ years of Case Management, Social Service, Health or Education employmentexperience required.
5.Valid WA State Drivers License & auto-liability insurance
6.Maintain a reliable personal vehicle and auto liability insurance. Must possess a fully insurabledriving record, indicating a history of safe and responsible driving.
7.CPR/First Aid certification and current Food Worker Card.
8.Exceptional organizational, problem-solving, and written and oral communication skills.
9.Strong proficiency in Microsoft Office and general computer skills.
10.Acknowledgment and understanding of systemic oppression and a commitment to systems change


11.Experience in building/maintaining partnerships between community organizations andcommunity coalitions working toward a common goal.
12.Experience working with high-risk children and families and the ability to work with individualsof diverse cultural/ethnic backgrounds.
13.Ability to develop relationships with, relate to, and motivate youth aged 13-21.
14.Experience working in a collaborative setting. Ability to work both independently and as ateam member, balancing multiple competing priorities.
15.Experience working autonomously with a high-level of self-direction and follow-through.
16.Personal qualities of integrity, credibility, creativity, and commitment to the organization’s
mission and the mission of ending youth houselessness.



PREFERRED QUALIFICATIONS & SKILLS:
1.Lived experience and cultural competency in Latinx communities and/or immigration systemnavigations.
2.Bicultural life experience, LGBTQ+ life experience, and other intersectional lifeexperience are considered assets to this position.
3.Experience in homelessness prevention and/or supporting populations experiencing unstablehousing, particularly through the McKinney-Vento Act.
4.Knowledge, training, education and/or lived experience in trauma-informed care, adversechildhood experiences and youth mental health.
