About the position:

The Community Health Worker is an integral part of the oncology care team. Community Health Workers liaise between patients and clinic personnel to connect patients to community resources and optimize a patient’s treatment journey. Community Health Workers are expected to assist with health literacy, cultural mediation, care access, and overall communication.

Shift Schedule:

  • Mon - Fri: 8am - 5pm

ESSENTIAL DUTIES AND RESPONSIBILITIES:

The Community Health Worker is a non-clinical member of the oncology care team that performs independent, interdependent, and dependent care actions while demonstrating personal professional accountability. The CHW is certified according to programs approved by the Texas Department of Health and Human Services and is expected to advocate for and coordinate quality patient care. Duties and responsibilities include but are not limited t0:

  • Facilitating communication between parties (patients, families, colleagues, and community-based organizations) as needed.
  • Preparing and educating patients prior to clinic visits
  • Following up with health management plans and goals
  • Developing trusting, supportive relationships with patients to serve as an advocate over the course of the cancer journey
  • Connecting patients to health-related services and community resources according to the health plan and positively identified social determinant of health needs
  • Assisting patients in accessing health-related services and community resources, such as accompaniment to appointments and aid with enrollment forms
  • Attending meetings as required
  • Documenting interactions and activities in the medical record/care coordination platforms

SKILLS REQUIRED: A qualified candidate is a professional, dependable, flexible, and self-motivated individual who:

  • Demonstrates competence in multicultural interactions
  • Communicates well with patients, medical staff, and office personnel
  • Exhibits compassion, accountability, responsiveness, and empathy during daily activities
  • Displays strong interpersonal and relationship-building skills
  • Breaks down complex information into manageable pieces
  • Builds and maintains relationships with relevant people and organizations
  • Maintains basic knowledge of the Houston area and given patient population
  • Exhibits basic knowledge of health insurance, including HMOs, PPOs, Medicare, Medicaid, etc.
  • Thinks critically in all situations
  • Possesses good computer skills and exhibits appropriate phone-etiquette
  • Organizes, plans, and prioritizes responsibilities well

EXPERIENCE AND EDUCATIONAL REQUIREMENTS:

  • Highschool Diploma or GED
  • Current Community Health Worker Certification
  • Bilingual Vietnamese preferred
