Description

The primary purpose of the Breast Health Nurse Navigator position is to assess and manage barriers to improve timeliness of care, quality of care, clinical outcomes, and patient experience.  Responsible for providing individualized assistance to patients, families, and caregivers as well as internal and external staff and providers.  The Breast Health Nurse Navigator serves as a central point of contact for identified patients confronted with BiRad 4 and BiRad 5 diagnostic mammograms.  Arranges services within the Breast Center and resources available through the community or service agencies, providing education regarding follow up needed.  The Breast Health Nurse Navigator will work collaboratively to coordinate patient care needs by communicating with the Oncology Nurse Navigator should the need for services of the Cancer Center.



Responsibilities

  • Assures proper patient identification

    • Identify patients who present with a high TC risk score (above 20%)

    • Ensure provider notification occurs

    • Educate patient on further screening options

    • Provide education concerning genetic testing

    • BiRad 4 and BiRad 5 patients


  • Provides patient care coordination

    • Assist in facilitating mammography appointments, diagnostic tests, and treatment procedures, ensuring timely access to necessary services

    • Explains the sequence of procedures to the patients

    • Educate screening patients should additional breast imaging be needed

    • Collaborates closely with radiologists and technologists to coordinate all components of clinical biopsy procedures, including scheduling, liaison activities with referring physician and radiologist, and patient education and follow-up

    • Streamlines process for patients by assisting with appointment scheduling with surgeon when necessary 

    • Assist with any procedural coordination


  • Advocacy for the patient

    • Serve as a strong advocate for patients, ensuring their voices are heard and their needs are addressed within the organization

    • Provide emotional support and coping strategies to help patients navigate the emotional challenges associated with a breast cancer diagnosis and treatment

    • Ensures smooth transitions between care modalities, facilities, and providers


  • Education, Community, and Support Services

    • Educates the patient and family regarding planned procedures, medications, and treatments

    • Have full knowledge of all funding options available to patients, ensuring coordination with Financial Assistance Program (FAP)

    • Manages information flow for all MS Dept of Health's Breast and Cervical Cancer Program, Memorial Foundation Breast and Cancer Fund and Memorial's Social Services Dept

    • Coordinates activities with Women's Center Coordinator and Marketing for Breast Cancer Awareness month in October of each year

    • Participates in health fair and other health related community events as required


  • Provide appropriate levels of communication

    • Excellent interpersonal skills, with the ability to establish rapport and build trusting relationships with patients and healthcare providers

    • Conducts follow-up conversations with patients and communicates any concerns, changes, or social needs to the appropriate physician or other care provider

    • Ensure timely and coordinated handoff to the Oncology Nurse Navigator as appropriate




Qualifications

Education Requirements



  • Required: A RN nursing degree (ASN or BSN)

  • Preferred:  Bachelor's Degree in Nursing


Licensure Requirements



  • Required:  Active, unencumbered RN license (Mississippi)


Experience Requirements



  • Required:  2 years of RN nursing experience

  • Preferred:  3+ years of experience as breast nurse navigator and/or breast patient educator


Skills and Core Competencies



  • Patient Advocacy

    • Serving as a liaison between patients and their care team


  • Care Coordination

    • Managing appointments, overcoming logistical barriers, and coordinating with specialists (radiologists, surgeons, oncologists)

    • Providing detailed information on disease processes and treatment options


  • Communication

    • Demonstrating strong, empathetic, and culturally sensitive communication skills



Work Environment:  Is generally in an office setting


Physical Demands:  90% of work is sitting and on computer, making phone calls
