Minimum 1+ years nursing experience required.

Description: We are seeking a skilled Registered Nurse Case Manager for a 13-week contract in Pleasant Hill, CA, to provide exemplary home health care. This full-time position offers the chance to make a meaningful difference in patients' lives.

Responsibilities:

Conduct comprehensive patient assessments and develop personalized care plans.
Deliver skilled nursing care, including wound care, medication management, and patient education.
Maintain accurate documentation using electronic medical records systems.
Collaborate with interdisciplinary team members to ensure coordinated care delivery.
Participate in on-call rotation and handle after-hours patient needs as required.

Requirements:

Active RN license in California.
Current BLS certification from the American Heart Association or the American Red Cross.
Previous experience in home health nursing required.
Preferred proficiency in OASIS and HCHB EMR systems.
Ability to work full-time hours, Monday through Friday, with possible on-call rotation.
