Hours: 1st shift, Monday through Friday | No Supervisory Responsibilities | No Travel | Pay commensurate with experience

PRN, day shift position in Rehab Services.

JOB SUMMARY:

Responsible for initial evaluation, orientation, implementation and follow-up of Phase I Cardiac and Cardiopulmonary Rehabilitation Programs.  Helps assess, plan evaluate and implement interdisciplinary base plan of care in all other phases.  Maintains vital sign records and symptomology documentation for all phases.

Experience:

• One year experience in Cardiac/Cardiopulmonary Rehabilitation Programs or equivalent experience preferred

Certifications:

• ACLS Certification


• BCLS Certification

PRIMARY JOB RESPONSIBILITIES:

1. Conducts initial evaluation of cardiac and pulmonary inpatients within 18 hours of the order to include medical history review, patient vital signs and patient symptomology.


2. Initiates and conducts cardiac and pulmonary inpatient therapy and education twice per day.


3. Assesses interdisciplinary base plan of care for cardiac and pulmonary patients.


4. Documents on patient records during each clinical visit.                                  


5. Conducts Phase II, III and pulmonary rehabilitation sessions as scheduled.


6. Assesses ability/inability to participate in and monitors reaction to exercise for patients.


7. Provides documentation and assessment to the patient’s personal physician within an appropriate clinical time frame.


8. Assesses cardiac and pulmonary rehabilitation patient education needs, maintains educational materials and plans and implements cardiac and pulmonary rehabilitation education once weekly.


9. Consults with and advises patients in designing an exercise, education, and lifestyle modification program to meet the ADL, recreational, vocational, and health needs of the patient within the limitation imposed by any disease and disease risk factor status.


10. Performs other duties as assigned.


11. Goals from previous performance evaluation were met.

Must be willing to receive all required vaccinations (i.e., flu shot, etc.).
