Job Description - Care Coordination Medical Assistant
Position Title: Medical Assistant – Care Coordination Department: Clinical Operations Status: FTE 1.0 Schedule: Full-time, Monday–Friday, 8:00 am – 4:30 pm
Position Summary
The Medical Assistant – Care Coordination supports providers and the Care Coordination team by managing clinical administrative tasks that promote continuity of patient care. Working from the corporate office, this position serves as a central point of communication between patients, providers, pharmacies, insurance companies, and outside agencies. This role does not provide direct patient care in a clinic setting but plays a vital role in ensuring timely coordination of patient needs and provider support.
Duties/Responsibilities
Manage and respond to patient portal messages, routing concerns to the appropriate provider when needed.
Assist providers with patient care coordination and follow-up activities.
Coordinate communication with pharmacies regarding prescription refill requests, medication questions, and prior authorizations.
Supports clinic systems and processes that help standardize the patientexperience.
Complete and process FMLA, disability, Social Security, and other medical paperwork for patients accurately and within required timelines.
Coordinate with internal departments and external organizations to facilitate patient care.
Maintain accurate documentation within the electronic medical record (EMR).
Maintain patient confidentiality and comply with HIPAA and organizational policies.
Perform other duties as assigned to support patient care and clinic operations.
Qualifications
High school diploma or equivalent required.
Current Medical Assistant certification or degree
Minimum of one year of experience in a medical office or care setting preferred.
Experience using an electronic medical record (EMR) system.
Strong organizational, communication, and time management skills.
Ability to manage multiple priorities in a fast-paced environment while maintaining attention to detail.
Excellent customer service and interpersonal skills.
Physical Requirements
Ability to sit, stand, and walk throughout the workday.
Frequent use of computers, telephones, and other office equipment.
Ability to occasionally lift up to 25 pounds.
Work Environment
This is a full-time, onsite position requiring attendance in the office five (5) days per week.
Standard business hours with occasional flexibility based on operational needs.
