Position Summary: Care Coordination Specialist
Thriving Texas Families Program- Grant funded by HHSC
Family Support Services of Amarillo is seeking a compassionate, organized, and relationship-driven Care Coordination Specialist to join our team through the Thriving Texas Families Program. This position plays a vital role in supporting pregnant women, parenting families, and caregivers with children ages 0–3 by helping remove barriers, strengthen family stability, and connect individuals to meaningful resources and support systems throughout the Texas Panhandle.
The Care Coordination Specialist will provide direct support and navigation services to families experiencing challenges related to poverty, trauma, mental health, transportation, housing instability, domestic violence, parenting stress, and access to healthcare or community resources. This team member will work alongside internal programs and community partners to ensure families receive holistic, trauma-informed, and family-centered care.
This position requires someone who can build trust quickly, communicate with empathy, maintain strong professional boundaries, and work effectively with individuals from diverse backgrounds. The ideal candidate is highly organized, solutions-focused, and passionate about walking alongside families during vulnerable and transformative stages of life.
Key responsibilities include conducting intake and needs assessments, developing individualized service plans, providing ongoing care coordination and follow-up support, assisting families with goal setting, connecting clients to community resources, documenting services, supporting outreach efforts, and collaborating with partner agencies and healthcare providers. The position will also assist with parent education initiatives, support groups, resource fairs, and community engagement activities connected to the Thriving Texas Families Program.
The Care Coordination Specialist must demonstrate strong communication skills, professionalism, flexibility, and the ability to work independently while remaining part of a collaborative team environment. Experience working with families, youth, survivors of violence, home visiting programs, behavioral health, case management, or community-based services is strongly preferred.
Qualifications:
• Bachelor’s degree in social work, psychology, human services, education, counseling, or a related field preferred
• Relevant experience in care coordination, family support, advocacy, or case management may substitute for education
• Experience working with at-risk families, pregnant women, infants, or young children preferred
• Strong organizational and documentation skills
• Ability to maintain confidentiality and professional boundaries
• Valid driver’s license and reliable transportation required
This position may require travel both locally and to targeted rural communities, outreach, evening events, and occasional home or community-based visits.
