The Care Coordination Supervisor is responsible for overseeing all staffing-related functions within the Care Coordination department. This position provides direct supervision, scheduling, performance management, and operational support to care coordination staff to ensure effective patient transitions, referral management, and continuity of care. The Care Coordination Supervisor works closely with clinical leadership and administrative departments and reports directly to the Organizational Development Manager.
Essential Duties and Responsibilities
Supervise and support all care coordination staff, including daily oversight of workflow, workload distribution, and performance.
Manage all staffing-related activities for care coordination, including scheduling, coverage planning, timekeeping coordination, and leave tracking.
Serve as the primary point of contact for staffing concerns, attendance issues, and workflow adjustments within the Care Coordination department.
Participate in hiring activities including interviewing, onboarding coordination, and training of care coordination staff.
Conduct performance evaluations, provide coaching, and initiate corrective action in alignment with organizational policies.
Ensure appropriate staffing levels to support referral management, care transitions, and patient follow-up activities.
Collaborate with clinic leadership, providers, and ancillary departments to support effective care coordination processes.
Monitor department productivity, workload trends, and staffing needs and communicate recommendations to the Organizational Development Manager.
Support implementation and adherence to departmental policies, procedures, and documentation standards.
Ensure compliance with applicable healthcare regulations, privacy requirements, and organizational standards.
Assist with reporting, audits, and quality improvement initiatives related to care coordination, staffing and performance.
Supervisory Responsibilities
This position has direct supervisory responsibility for care coordination staff. Responsibilities include staff scheduling, performance management, coaching, training oversight, corrective action, and fostering a collaborative and accountable team environment.
Qualifications
Associate or bachelor's degree in healthcare administration, nursing, social work, or a related field preferred.
Experience in care coordination, case management, or healthcare operations required. Previous supervisory or staffing management experience required.
Knowledge of referral processes, care transitions, and interdisciplinary healthcare coordination.
Strong organizational, communication, and leadership skills.

Work Environment
