Care Coordination Supervisor

Position Summary:

An organization dedicated to promoting independent living and supporting individuals with disabilities is seeking a dedicated and motivated Care Coordination Supervisor to join our team. The Supervisor will exemplify our mission and vision, oversee Care Coordinators, manage performance, and handle new referrals. This role involves ensuring program eligibility, maintaining accurate consumer counts and billables, and occasionally covering other employees' responsibilities.

Minimum Qualifications:

- Experience may be substituted for education.

- Consistent punctuality with a flexible schedule.

- Timely and error-free documentation.

- Proficient communication skills (oral and written).

- Ability to work well under pressure.

- Personal means of transportation necessary.

Preferred Qualifications:

- Bachelor's degree in human services or a related field.

- Personal experience within the disability community.

- Experience with Home and Community-Based Services.

Essential Functions:

- Provide coaching for Care Coordinators and assist them in managing other personnel.

- Incorporate consumer feedback into evaluations and conduct performance reviews.

- Manage referrals, demographic information, and ensure timely service starts.

- Address billing issues and ensure consumer eligibility.

- Utilize Microsoft Suite programs.

- Monitor and fulfill consumer authorizations.

- Ensure quality of service through regular consumer meetings, phone contacts, and documentation reviews.

- Assist coordinators daily to understand and meet consumer and attendant needs.

- Coordinate compliance-related documents and incidents.

- Be available for on-call issues and ensure communication tools are functioning properly.
