Job Description - Care Coordinator


Our mission at Lifelong Access is to connect individuals with developmental disabilities to their community by helping them break down barriers through a lifetime of meaningful supports. From birth to end of life, Lifelong Access provides a continuum of services and supports for families, children, teens, and adults including residential housing, vocational development, supported employment, high school transition, behavioral health counseling and services, and a wide variety of pediatric therapy supports. Lifelong Access is seeking a Care Coordinator to act as a primary point of contact for clients to help navigate the various care services and related scheduling, insurance authorizations and other necessary documentation. This role will ensure smooth operations, along with providing excellent customer service to patients and their families. This position requires strong organizational, communication, and problem-solving skills. As the largest provider of pediatric therapy services in McLean County, we are excited to continue growing our program with an exceptional team of professionals. Pay:               
$24–$25 per hour (based on experience)
Job Type:       Full-time (5 days per week with some flexibility in hours; preferred working hours are 8 AM–4 PM or 10 AM–6 PM)

Primary responsibilities include:
  • Point of contact for active clients in Pediatric Therapy (Speech, Physical Therapy, Occupational Therapy, and Early Intervention.
  • Point of contact for master scheduling.
  • Directly handles ongoing insurance authorizations & medical records.
  • Coordinates with intake coordinator.
  • Shares front desk duties with 1-2 other support staff; rotational basis.
  • May handle quarterly department reporting.
  • Assist Site Director with community engagement & parent connections, which will also include a variety of clerical tasks.
Qualifications:
  • Minimum of high school diploma or GED.
  • Minimum 1-year previous experience working in an office setting, preferably in a medical environment.
  • Excellent interpersonal skills, including ability to provide positive customer service experience. 
  • Demonstrated ability and willingness to promote a positive work environment.   
  • Ability to work with a high level of independence in prioritizing responsibilities and adapting to changing conditions.  
  • Ability to work with distractions and interruptions in the work environment. 
  • Strong organizational skills.
  • Valid Illinois driver’s license with safe driving record.
  • Current proof of automobile insurance.
Working Conditions    
While performing the duties of this job, the employee will be in an inside office environment.  The noise level in the work environment is usually moderate.

While performing the duties of this job the employee will have prolonged periods of sitting at a desk and working on a computer. Employee is regularly required to talk and hear. Specific vision abilities required by this job include close and distance vision. The employee is frequently required to stand, walk; sit; use hands to fingers, handle, or feel; and reach with hands and arms. Occasionally is required to stoop, kneel, crouch, or crawl.  The employee must be able to lift to 15 pounds frequently, and up to 25 pounds occasionally.
