Care Coordinator

Care Coordinator's coordinates continuity of patient care with external healthcare organizations and facilities including from the primary care provider to a specialty care provider. Responsible for verifying patient eligibility and benefits and acquiring prior authorization for services being rendered to the patient to maximize positive financial outcome for the facilities.

Role Specific Responsibilities:

• Provide data management, coordination, and patient outreach as needed for specific target patient populations.
• Contributes to a positive experience for patients and families through courteous telephone interactions, accurate and expeditious routing, as well as referral to appropriate clinical staff  when necessary
• Contributes to the  teamwork within and between departments. Regularly attends and participates in meetings with coworkers and practice staff. Provides  constructive ideas, suggestions, and feedback in a positive manner 
• Coordinates continuity of patient care with external healthcare organizations and facilities, and referrals from the primary care provider to a specialty care provider 
• Collaborates professionally with other members of the healthcare team
• Serves as a liaison between the clinic, hospital and/or other appropriate community resources 

Required:

· High School Diploma or GED 

· Strong clinical, ethical and moral character references

· Language skills adequate for high-level written, interpersonal, and telephone communications in American English
