We are seeking a full-time Care Coordinator to join our Respite Program Team. Respite is a supportive service designed to give primary caregivers and youth living with mental and behavioral health concerns a pause from their regular routines in a group setting using evidence-based practices and recreation therapy modalities.

To Qualify:
- High School Diploma required, BA in counseling or related field preferred.
- Registered Agency Affiliated Counselor Credential or ability to become registered within 60 days required.
- Comprehensive knowledge of community resources and public systems.
- Minimum three years of working with youth and families.
- Exceptional communication and interpersonal skills.
- Community Health Worker certification preferred.
- Must have strong multitasking and organizational skills.
- Ability to work in a fast-paced environment and make ethical and safe decisions.
- Ability to manage individual stress and perform essential job functions in a safe and professional manner.
- Must be at minimum 21 years of age.
- Ability to pass criminal background check.
- Valid driver's license and acceptable driving record, per organizational standards.
- New employees with out-of-state licenses must obtain a Washington State Driver’s License within 30 days of employment*
- Current certification as a Community Health Worker preferred
- Minimum of three years’ experience working with youth and families preferred
Core Responsibilities Include:

- Care Coordination & Case Management: Establish and sustain regular contact with Respite clients and their families to monitor progress and identify and address potential barriers within systems.
- Contribute to an Interdisciplinary Team: Work closely with an interdisciplinary team that includes roles such as mental health therapists, peer support specialists, and certified recreation therapy specialists to deliver safe and quality client care.
- Facilitate Client Enrollment and Orientation: Support the seamless enrollment and orientation of new clients into programming, ensuring they understand available services and resources.
- Crisis Intervention Support: Respond to and effectively navigate crisis situations in collaboration with a mental health therapist, ensuring safety and support for clients.
- Guide Resource Implementation: Provide direction and guidance to clients and families in accessing and utilizing community resources, helping them achieve desired outcomes throughout the treatment process.
- Participate in Group Sessions: Support, facilitate or co-facilitate group sessions as needed in order to maintain a safe staff to client ratio and foster a supportive group environment.
- Community Engagement: Actively engage with community partners and stakeholders to promote programs and facilitate referrals and enrollment in services. Represent Excelsior in various community settings contributing to organizational goals and community presence.
- Participate in Team Meetings: Attend and contribute to regularly required meetings, sharing insights and collaborating on client care strategies.
- Continuous Professional Development: Complete all required training and demonstrate proficiency in skills essential for the role, ensuring high standards of care.
- Conduct Trainings: Organize and conduct necessary trainings as directed by the program manager, fostering professional development within the team.
- Uphold Professional Standards: Maintain professionalism and cohesion in all communications while safeguarding client confidentiality.
- Adhere to Ethical Standards: Uphold organizational ethical standards, policies, procedures, and the mission, vision, and values of Excelsior.
- Provide Client Transportation: Ensure safe transportation for clients to program activities as needed using company vehicles.
