Job Description - Care Coordinator

Our Care Coordinator will be a vital team member in the daily operations of our company. Our Care Coordinator will ensure that our clients receive the care they need from our caregiving team and oversee our caregiver training program. Individuals with a background in caregiving who enjoy mentoring and nurturing individuals for professional growth and who possess excellent client interaction skills, are able to multi-task, and work independently will love this job and are encouraged to apply!


 


$20-$23 Hourly


 


Schedule/Work Setting



  • Monday-Friday 8:00 am-4:30 pm.

  • Must have ability to work out of both NWI offices (Crown Point & Chesterton) as well as throughout our service area.


 


What Will Our Care Coordinator Recruiter Do?



  • Communicate with clients and their families to assess the quality of Senior Helpers services and the client and family satisfaction level.

  • Perform supervisory client visits to ensure care plans are implemented correctly and all job duties are performed in a manner that is satisfactory.

  • Lead caregiver onboarding and training programs.

  • Update client care plans and communicates changes to family members, staff, and caregivers.

  • Performs skill assessments for all caregivers and provide follow-up training as needed.

  • Perform client assessments and reassessments utilizing our proprietary Life Profile technology.

  • Collaborate with their supervisor to create and implement caregiver retention and recognition programs.

  • Develops great relationships with caregivers and clients & looks for ways to improve quality and overall service.


 


What We’re Looking For in Our Care Coordinator:



  • College Degree – Associates or Bachelors (required)

  • Certified Nursing Assistant (CNA) or equivalent work experience in lieu of license (required)

  • Minimum of two years in an office setting with supervisory experience. Experience in workforce training highly desired.

  • Professional experience in the field of customer service.

  • Proficiency with Microsoft products

  • Strong administrative skills

  • Team Player, excellent verbal and written communication skills. Ability to multi-task and work independently.

  • Proficiency with Microsoft products

  • Ability to pass a criminal background check and MVR check
