The Role

As a Care Coordinator, you're often the first and last voice a patient hears — the person who sets the tone for their entire PHW experience. You'll manage front desk operations, patient scheduling, medical records, and member support, all while embodying our +IMPACT approach to service. 

What You'll Do

• Greet and check in patients with warmth, professionalism, and genuine care


• Manage scheduling for in-office and virtual visits across providers and services


• Maintain accurate, organized medical records and ensure HIPAA compliance


• Answer phones, respond to patient inquiries, and route messages to the appropriate team member


• Process membership enrollments, renewals, and billing questions


• Support supplement fulfillment — assisting members with orders, pickup, and product questions


• Collaborate with the Medical and Provider Teams to ensure smooth patient flow


• Help identify opportunities to improve office processes and patient experience


• Represent PHW's mission and values in every patient and teammate interaction

What We're Looking For

• Genuine warmth and a service-first mindset — you love helping people feel taken care of


• Strong organizational skills and attention to detail


• Comfort with multitasking in a fast-paced clinic environment


• Clear, friendly communication — in person, by phone, and in writing


• Comfort learning and using EHR/scheduling software (training provided)


• Prior experience in a medical office, customer service, or hospitality role preferred but not necessary


• A collaborative, coachable attitude — willing to give and receive feedback


• Passion for functional medicine, wellness, or proactive health a plus
