Title: Care Coordinator
Reports To: Office Manager and CEO
Job Overview: We are seeking a dedicated Care Coordinator to assist us with patients enrolled at Georgina Home Care. You will be responsible for consulting with patients and determining their needs, developing care plans, coordinating patient-care services, and ensuring quality assurance.
The successful employee will be compassionate, knowledgeable about healthcare practices, and able to provide exceptional customer service.
Responsibilities and Duties:
Provides coverage for call-out, new clients, sick, vacation, and long-term leave.
When a change of caregiver is done, do a case acceptance form.
Communicate between caregivers, client, case manager, and regarding changes to client health, schedule, and caregiver.
Hospitalization; report to everyone in the client's care team and fill out a form.
Supervise schedules and ensure daily clock-ins and clock-outs are done, and schedules are confirmed.
10 Day courtesy call when there is a staff change
Communicate with all departments about caregivers and times for corrective action and payroll purposes.
Complete Intakes for new clients
Conduct Home Visits for clients
Sandata Maintenance: Update schedules and ensure caregivers are clocking in and out.
Qualifications:
Associate degree in related fields or three years of experience.
Bilingual Spanish Speaking
Driver License and reliable transportation.
