Ovation Home Care is actively seeking a  Care Coordinator to provide the critical link between our office team and our caregivers where they serve our clients in residential and facility settings.

The Care Coordinator is a liaison between our field staff (CNAs and Home Health Aides), their clients/families, and our office.

Because of the nature of this opportunity, you must be CNA/HHA with 10+ years of experience to be considered.

• Field Visits (2-3 per month per client)


• Work with other office team members to coordinate various aspects of a client's start of care.


• Start of care "Meet & Greet" (where you'll set up the caregiver for success by introducing them to the client and reviewing the care plan). 


• Recognize and pursue opportunities to promote client engagement.


• Identify when the client might benefit from a different schedule or additional hours.


• Assist Client Experience Manager with client related administrative tasks.


• Schedule and attend any necessary pre-discharge rehab training. 


• Fill in for client shifts as needed.

• CNA or Home Health Aide with 10+ years of experience.


• Motivated self-starter

• Highly organized and able to handle change without being flustered


• Computer/tech savvy 


• Friendly on the phone


• Ability to write and speak professionally


• Desire to build trusting relationships with our clients and caregivers 


• Have some experience with duties listed above 


• Be able to lift 25 pounds 


• Ability to speak calmly in stressful situations 


• Complete and pass a Level 2 nationwide criminal background check, DMV check. 


• Possess a valid driver's license and auto insurance with you listed as a covered driver
