POSITION TITLE: Care Coordinator for Free Clinic of Fulton County

REPORTS TO: Medical Director of Free Clinic

WORKS IN COLLABORATION WITH: Health care/medical providers (primary care physicians, physician assistants and nurse practitioners, nursing staff, interpreters and other volunteers)

POSITION SUMMARY: This is a contract position. Hours can vary monthly. Clinic is held at the North Medical Office Building once a month on the third Tuesday 5:30 pm until finished at present. Ideally would be held at least twice monthly. The Care Coordinator ensures that patient care coordination is implemented by managing patient caseloads per clinic, ensuring adequate staffing, and keeping patient charts up to date. Schedules testing and referrals. Ensures all patient documentation after clinic. Contacts patients for follow up as needed.

Keeps abreast of opportunities for direct and support services and refers clients to needed services. Occasional travel to meetings is required.

· HIPAA knowledge and compliancy

· Ability to build relationships with different types of people

· Some grant work may be required

Education and experience:

· RN/LPN ideally with physician office experience

· Strong understanding of cultural competency with the target population

· Computer literacy

· Compensation commensurate with experience, education and skills.
