JOB SUMMARY: The Care Coordinator I (CCI) serves as an entry level team member responsible for post-discharge care transitions and care coordination. This role includes both client-facing responsibilities and administrative coordination to ensure people we serve have the resources and education needed to avoid readmissions and be safe in the community. . 

RESPONSIBILITIES: 

• Monitor alerts via RPM platforms and escalate concerns to clinical staff. 

• Engage patients post-discharge to confirm follow-up and service completion. 

• Support referral processing and patient outreach tasks. 

• Document communication and maintain accurate patient records. 

• Assist with care coordination tracking and reporting tools. 

QUALIFICATIONS: 

• High school diploma or equivalent required. 

• Bilingual preferred – but required for some roles. 

• 6+ months of healthcare or customer service experience preferred. 

• Medical Assistant certification preferred. 

• Strong communication and organizational skills. 

• Basic understanding of medical terminology and patient care processes.
