Care Coordinator III (LPN) (MSW) (BSW) - Bilingual

About the Role  

Join our Ensemble CIN team in Las Cruces, NM, as a Care Coordinator III. You’ll play a pivotal role in overseeing complex care coordination and clinical assessment for patients transitioning across settings, all within a small, collaborative team environment.

Key Responsibilities

• Monitor remote patient monitoring (RPM) alerts and escalate clinical concerns promptly.
   
• Conduct thorough patient assessments and create individualized, patient-centered care plans.
   
• Coordinate with interdisciplinary teams—nurses, social workers, therapists—to ensure seamless delivery of clinical and social services.
   
• Educate patients and caregivers on treatment plans, medications, and follow-up requirements.
   
• Ensure all documentation is accurate, timely, and compliant with regulatory standards.
   
• Mentor and support junior Care Coordinator I/II staff, sharing best practices and clinical insights.
   
• Track patient outcomes using documentation platforms and outcome-monitoring tools.

What We’re Looking For

• Licensure & Experience: Active LPN BSW, MSW license required, plus three or more years in care coordination, case management, or clinical practice.
   
• Clinical Judgment: Advanced problem-solving and decision-making skills to manage complex patient scenarios.
   
• Communication: Exceptional verbal and written communication strengths in both English and Spanish required.
   
• Technical Proficiency: Comfortable with electronic documentation systems and RPM platforms.
   
• Collaboration: Demonstrated ability to work closely with multidisciplinary teams in a fast-paced environment.
   
• Leadership: A passion for mentoring and guiding less-experienced staff members.
