Care Coordinator - Pathlight HOME

Employment Status: Full-Time / Non-Exempt

Pay: $18.03 per hour ($37,502.40 annually), with higher compensation based on experience and education.  

About the Position

IMPOWER is seeking a Care Coordinator to support behavioral health services for individuals served through the Pathlight program. This on-site position plays an important role in helping individuals access and stay connected to care by coordinating clinic visits, outpatient therapy, medication needs, referrals, and telehealth services.

As a Care Coordinator, you’ll serve as a primary point of contact for individuals served and work closely with clinical providers, Pathlight Case Managers, Clinic Assistants, pharmacies, funding sources, and community partners to help ensure timely access to services and continuity of care.

Key Responsibilities

• Process referrals and establish new records in the electronic health record (EHR), ensuring required consents and documentation are completed
• Coordinate in-person and telehealth appointments and provide technology assistance as needed
• Track prescriptions, maintain pharmacy information, coordinate required documentation, and assist with prior authorizations
• Verify insurance and funding eligibility and complete required grant-related documentation
• Maintain accurate appointment, demographic, insurance, funding, and pharmacy information
• Coordinate laboratory services, medical record requests, community referrals, and referrals to higher levels of care
• Conduct Psychosocial Rehabilitation (PSR) groups for individuals in the Pathlight program in accordance with applicable program and Medicaid requirements
• Assist with FAR assessments, track required assessment timeframes, and communicate with outpatient providers regarding reassessments
• Collaborate with clinical staff and Pathlight Case Managers to identify and address barriers to care and connect individuals with appropriate services and community resources

Qualifications

• Bachelor’s degree in a human services or related field required
• Bilingual skills are a plus
• At least one year of experience in a fast-paced medical office, behavioral health, psychiatric practice, or similar healthcare environment
• Strong organizational, communication, judgment, and problem-solving skills
• Ability to manage multiple priorities in a fast-paced environment
• Knowledge of de-escalation techniques and professional boundaries
• Comfortable using computers, Microsoft Office, electronic health records, and other technology
• Bilingual skills are a plus
