Job Description - Care Coordinator (Scheduler)



This Is Not an Entry-Level “Answer the Phone” Job.


This is a mission-critical operations role inside one of Utah’s fastest-growing home care companies.


At Interim Home Care Utah, we don’t tolerate chaos.
We build systems.

And we are looking for a Care Coordinator who thrives in responsibility.


 


 


The Role


As a Care Coordinator, you are the heartbeat of operations.


If a shift goes unfilled — families suffer.
If communication is sloppy — trust erodes.
If schedules aren’t accurate — the entire machine breaks.


Your job is to make sure that never happens.


 


 


What You Will Be Accountable For




  • Shift fulfillment – Every shift covered, every time




  • Maintaining schedule accuracy – No surprises, no confusion




  • Clear & timely communication – With caregivers, facilities, and internal team




  • Building facility relationships – Professional, responsive, trusted




  • Maintaining updated points of contact – Always knowing who to call




  • Participating in operations calls & team alignment




  • 24-hour on-call rotation (shared among team)




  • In person client visits as needed




 


You will coordinate schedules based on client needs and caregiver availability, ensuring shifts are filled promptly and accurately


You will serve as the primary point of contact for caregivers and facilities regarding assignments, schedule changes, and expectations


This role requires decisiveness, urgency, and ownership.

Schedule & Compensation


Monday–Friday, 8:30 AM – 5:00 PM (30-minute break)
• Weekly pay at $20/hour + depending on experience


• Full-time benefits including:




  • Paid time off

  • Participation in shared 24-hour on-call rotation
