CARE COORDINATOR-SBHI

General Summary/Responsibilities:\n\nThe Care Coordinator functions as a member of a multi-disciplinary care team providing services for persons with an alcohol and/or other drug Substance Abuse Disorder diagnosis. The primary responsibilities of the care coordinator include: 1) Support adherence to the patient\u2019s treatment and recovery plan; 2) Manage successful care transitions; 3) Engagement in care; 4) Services may be delivered by phone, telehealth, face-to-face at the office, in the home, or in the community. This position requires expertise in transition planning and process, resource allocation, team management and communication skills\n\nEducation\n\nMinimum Level of Education Required: Associate degree\n\nAdditional requirements:\n\nType of degree: Social Work, Counseling or related field\n\nArea of study or major: Human and/or social services\n\nPreferred educational qualifications: N/A\n\nPosition specific testing requirement: N/A\n\n \n\nLicensure/Certification/Registration\n\nCDCA or Licensed Chemical Dependency Counselor II (LCDC II) or higher or a Bachelor Level LSW or LPC.\n\n \n\nExperience\n\nMinimum Level of Experience Required: No prior job-related work experience\n\nPrior job title or occupational experience: 3-6 months experience in the mental health and/or drug and alcohol field\n\nPrior specific functional responsibilities: N/A\n\nPreferred experience: 1-2 years\u2019 experience in the mental health and/or drug and alcohol field\n\nOther experience requirements: N/A\n\n \n\nKnowledge/Skills\n\n\u25aa Working knowledge of mental health, drug and alcohol, and other community agencies and resources.\n\n\u25aa Excellent interpersonal skills, critical thinking and ability to communicate in writing.\n\n\u25aa Excellent organizational skills.\n\n\u25aa Must be physically and mentally able to competently deal with verbally and physically aggressive patients\n\n\u25aa Computer keyboarding or typing skills recommended\n\n\u25aa Knowledge of the behavioral health field and best practices\n
