Job Description - Care Integration Coordinator

ORGANIZATIONAL BACKGROUND:


The Central California Asthma Collaborative (CCAC) is a non-profit organization dedicated to advancing health equity and environmental justice by empowering central California communities through services, research, advocacy, and data driven solutions to reduce the burden of asthma.



POSITION SUMMARY:


The Care Integration Coordinator, under the direction of the Director of Community Health and Director of Data Analytics, provides internal assessment and coordination between the Enhanced Care Management (ECM) program and Comprehensive Asthma Education and Remediation Services (CARES) program. This position is responsible for managing internal referrals between ECM and CARES, analyzing program data, and generating detailed reports to inform decision-making and improve service quality and delivery.


 


RESPONSIBILITIES:


·       Coordinate and track internal referrals between ECM and CARES programs to ensure timely and appropriate service delivery.


·       Analyze program data, generate performance and operational reports, and identify trends to support quality improvement efforts.


·       Review and process MIF files received from managed care plans, ensuring data integrity and actionable follow-up.


·       Prepare and review RTF files sent to managed care plans, verifying accuracy and compliance with contractual requirements.


·       Work closely with ECM and CARES managers to support program operations, including data review, meeting preparation, and documentation.


·       Collaborate with the management teams to streamline workflows and enhance coordination between programs.


·       Maintain accurate and organized records for audits, reporting, and program evaluation.


·       Stay updated on ECM and CARES program requirements and assist in implementing policy and procedure updates as needed.


·       Participates in Quality Assurance (QA) projects within ECM and CARES focusing on performance improvement and transformation aimed at improving the program for the populations served.


·       Other duties may be assigned.

TRAVEL:



  • To assigned CCAC sites and various meetings as needed; approximately 20% of the time.


QUALIFICATIONS AND SKILLS:



  • Bachelor’s degree in Public Health, Social Work, Health Administration, or a related field (Master’s degree preferred)

  • At least one year of experience in program coordination, case management, data analysis, or a similar role in a healthcare or community health setting.

  • Proficiency in data management and data analysis software; experience with healthcare referral platforms preferred.

  • Experience working with community-based organizations preferred.

  • Ability to work independently, cooperatively and responsibly in a fast-paced environment.

  • Demonstrated commitment to community education, environmental and social justice, and healthcare access.

  • Demonstrated attention to detail, organization, reliability, flexibility and professionalism.


DRIVING AND LICENSE REQUIREMENTS:


Must possess a valid CADL and applicable CA mandated auto insurance.


 


PHYSICAL REQUIREMENTS: 


Lift up to 20 (twenty) pounds. 


 


AUTHORITY AND SUPERVISORY CAPACITY: No supervision.


 


ACCESS TO SENSITIVE INFORMATION: Must maintain confidentiality of personal client and staff information exposed to when working in files and preparing reports.
