Position Summary:
Reporting to the Nurse Supervisor of Care Management, the Care Management Coordinator is responsible for supporting the coordination and delivery of care management services for high-risk and complex patients. This role serves as an operational and clinical support resource to ensure effective care coordination, patient engagement, and timely follow-up across the continuum of care.
The Care Management Coordinator works closely with nurses, providers, and interdisciplinary teams to support population health initiatives, reduce care gaps, improve patient outcomes, and enhance access to services through proactive outreach and coordination.

Qualifications & Skills:
· High school diploma or equivalent required; completion of an accredited Medical Assistant program or related healthcare training strongly preferred
· Medical Assistant certification (CMA, RMA, or equivalent) preferred
· Minimum of two (2) years of experience in an ambulatory care, community health, or care coordination setting required
· Experience with patient outreach, scheduling, and care coordination strongly preferred
· Familiarity with population health concepts and care management workflows preferred
· Experience with EHR systems (Epic preferred)
· Strong organizational and time management skills with ability to manage multiple tasks
· Strong communication and interpersonal skills with ability to engage diverse patient populations
· Ability to work collaboratively within interdisciplinary teams
· Basic understanding of healthcare systems, insurance, and community resources preferred
· Commitment to patient-centered care and improving health outcomes
Physical Requirements:
· Must be able to stand or sit for prolonged periods (at least 50% of the time)
· Ability to lift up to 25 pounds and load onto shelves
· Visual acuity sufficient for frequent reading and computer use

Pay Range: $26.00 to $40.51 Hourly. Compensation is commensurate with years of experience.
