Job Description - CARE Manager

WHITMAN COLLEGE

POSITION PURPOSE
Reporting to the Assistant Dean of Students, this non-clinical role provides goal-oriented and strengths-based assessment, intervention, and coordination of services to students experiencing academic, personal, or medical challenges. The Care Manager helps remove barriers to success and enhance holistic well-being, serving as a key resource for referrals, participation, and intervention delivery through the campus CARE Team and the Counseling Center. 


PRINCIPAL ACCOUNTABILITIES


Case Management Responsibilities: (25%)



  • Provide outreach to students referred by the CARE Team or the Counseling Center to schedule appointments.

  • Assess students holistically to identify needs and appropriate support. 

  • Provide follow-up appointments to coordinate action plans, referrals, and connections to on- and off-campus resources.

  • Communicate and collaborate with appropriate staff and resources.

  • Prepare letters of consideration for students as needed.

  • Maintain accurate case notes in the Advocate System.


CARE Team Responsibilities: (25%)



  • Prepare case notes on current students, review new referrals, and actively contribute to discussions in CARE Team meetings. 

  • Refer students to the CARE Team as appropriate when providing case management support.

  • Support the CARE Team by delivering interventions and submitting Care Reports through the Advocate System.


Co-Supervisor of Peer Academic Leaders (PALs) Responsibilities: (20%)



  • Co-supervise the Peer Academic Leaders (PALs) pilot program in partnership with Residence Life and Housing to ensure its success.

  • Provide supportive measures to help the PALs foster academic success and create a welcoming, inclusive environment that promotes community and well-being.


Outreach and Awareness Responsibilities: (15%)



  • Develop marketing, awareness, and education campaigns to promote case management and services offered through the PALs Program.  

  • Create tailored educational presentations to meet the needs of diverse audiences.

  • Collaborate with campus offices to ensure cohesive messaging and program integration.


Departmental Responsibilities (15%)



  • Adapt to evolving institutional projects, strategic initiatives, or other duties as assigned by the supervisor.

  • Assist with compiling end-of-semester and end-of-year reports.

  • Support strategic planning efforts through development and implementation. 

  • Contribute to assessment initiatives to evaluate case management effectiveness.

  • Serve on committees as assigned.


REQUIRED KNOWLEDGE, SKILLS & ABILITIES


Must be able to explain and/or demonstrate possession of the knowledge, skills, and abilities to safely perform the essential functions of the job, with or without reasonable accommodation. Specifically,



  • Demonstrated knowledge, sensitivity, and ability to support students from diverse ethnic, cultural, and socio-economic backgrounds, including LGBTQIA+ students and students with disabilities. 

  • Experience with organizational development and outreach. 

  • Strong problem-solving skills with a creative approach.

  • Proven ability to work autonomously while demonstrating effectiveness, and sensitivity. 

  • Ability to provide empathic support to students navigating challenging times.

  • Excellent interpersonal and communication skills and the ability to collaborate within a multidisciplinary team.


MINIMUM QUALIFICATIONS



  • Bachelor's degree.


PREFERRED QUALIFICATIONS



  • Experience providing interventions and developing treatment plans for clients.

  • Specialty skills or certification in mental health and wellness that can be offered to students in a group setting. 

  • Membership in HECMA.

  • Strong multicultural competence with an interest or expertise in outreach and preventative programming.
