PURPOSE OF THE POSITION: The Care Managers service those who are enrolled in Medicaid, have Two or more Chorionic Conditions, HIV/AIDS, Sickle Cell, Serious Emotional Disturbance (SED), Serious Mental Illness (SMI) or Complex Trauma; ages 0-21 (Childrens Program) & 18 to end of life (Adult Program). The Care Manager plays a critical role in achieving the overall goals for coordination, integration and partnership in the delivery of care to children, adults and families with complex needs.

SPECIFIC RESPONSIBILITIES
The Health Home Care Manager delivers all six NYS Department of Health (NYS DOH) Health Home Core Services, including:
Comprehensive Care Management
Care Coordination and Health Promotion
Comprehensive Transitional Care
Individual and Family Support
Referral to Community and Social Support Services
Use of Health Information Technology to Link Services
Additional responsibilities include:
Conducting NYSDOHârequired assessments and developing compliant, personâcentered Plans of Care
Coordinating medical, behavioral health, substance use, and social service supports
Engaging members through outreach, advocacy, and ongoing followâup
Collaborating with hospitals, providers, Manage Care Organizations (MCO), and community partners
Maintaining accurate, timely documentation in NYSDOHâapproved systems
Meeting Health Home productivity, engagement, and quality benchmarks
Participating in case conferences, audits, supervision, and training
RELATIONSHIP WITH OTHERS & IN THE WORKPLACE
Demonstrates professionalism, maintains appropriate boundaries, and follows the agency’s

WORKING CONDITIONS
This is a hybrid office- and field-based position requiring regular travel throughout Suffolk and Nassau Counties and select boroughs of New York City. Travel may occur multiple times per week based on program needs.
The role includes scheduled in-office days, with meetings and trainings held in person at our Brooklyn or Dix Hills locations, as well as virtually as needed. While every effort is made to assign care managers to cases within their designated catchment area whenever possible, flexibility to travel for service delivery is required. Applicants must reside in New York State.
QUALIFICATIONS
Care Manager must have one of the following:
A Masters with one year of relevant experience, OR
A Bachelors of Arts or Science with two years of relevant experience,
A License as a Registered Nurse with two years of relevant experience, OR
Relevant experience includes care coordination, case management, social services, behavioral health, medical care navigation, or community-based services.
All Health Homes Serving Children (HHSC) Care Managers must:
Complete Children Adolescent Needs Assessment (CANS)NY training
Be CANSâNY certified annually
Meet exam score requirements:
Care Managers: minimum 70%
Supervisors: minimum 80%
Bilingual Preferred ( Spanish )
