Care Manager - CBHC (Bilingual: Spanish)

Eliot is seeking motivated Bilingual (Spanish) Care Managers to join our Community Behavioral Health Center (CBHC) which serves as a single entry point for mental health and substance use services for youth, families and adults. The ideal candidate will provide in-home support and services to clients, primarily focusing care coordination and ensuring that treatment goals are culturally competent, realistic, attainable and promote recovery.

Responsibilities:

• Provide client support focusing on problem-solving, skills training, behavior modeling, optimism, and encouragement


• Deliver interventions in the office, virtually, in the community, and at clients' residences based on client preferences, needs, and clinical direction


• Enroll eligible individuals in care management programs as needed. 


• Assist the team and clients in ongoing assessment of symptoms and responses to interventions


• Contribute to the development of treatment plans as part of the client's treatment team


• Ensure treatment goals are culturally competent, strength-based, realistic, attainable, and promote recovery


• Ensure each individual is actively involved in their treatment planning process, and their preferences are incorporated


• Coordinate and ensure timely face-to-face contact with enrollees in the community following inpatient discharge.

Qualifications:

• Bachelor’s degree in Psychology, Social Work or related field


• 3 year experience working with adults with mental illness and/or substance abuse


• Fluent in Spanish and English


• Valid Massachusetts Driver's License

Schedule: Monday-Friday, 9am-5pm (2 days remote/3 days on-site)
