You’ll
Do:
Provide
information and education to help
I
ndividuals
understand the Medicaid waiver process
Create
a
n individual support plan
based on the
strengths, needs, goals, and desires
of the
I
ndividuals we serve
Help
I
ndividuals
access services and supports they need to meet
the
ir goals
Facilitate
regular
community-based
team meetings to review plan
of care
and collaborate with service providers as needed
Who
You’ll

(A&D)
and Traumatic Brain Injury
(TBI)
Medicaid Waivers
What You Need:
A
valid driver's license
,
car insurance
, and reliable transportation
A
B
achelor’s
D
egree in
H
uman
S
ervices
or an
A
ssociate
D
egree in
N
ursing and at least 12 months of experience with the aging/disabled population
OR
A
n
A
ssociate
D
egree
with 4 years of direct experience with the aging/disabled population
T
he ability to pass thorough background screens
D
esignated home office setting with computer, internet service, and smart phone
