Care Manager, Mental Health

Envision Unlimited is a large non-profit agency committed to providing services for adults with intellectual/developmental disabilities and/or mental illness. Our programs include, but are not limited to, ABA Therapy for Children, Community Day Program, Community Living, Mental Health, Employment and Career Services, and Foster Care.

Purpose:

Provide care coordination assistance to seniors, clients and families with mental health conditions in the areas of medical, psychiatric, social services to enhance quality of life and achieve positive outcomes. 

Essential Responsibilities, include but are not limited to:

• Work with therapist to develop the clients service plan
• Assist clients to apply for benefits such as Medicare, Medicaid, SNAP, Paratransit and other needed resources. 
• Provide linkage case management to primary and specialty care including psychiatry together with social determinants of health. 
• Assist clients to coordinate healthcare needs such as annual physicals, lab work, dental and eye care. 
• Work with client and family members to ensure client attendance at medical appointments and compliance with physician’s orders
• Secure medical transportation services, as needed
• Maintain medical documentation in the electronic health record system
• Attend client staffings as part of the multi-disciplinary team.

EDUCATION, SKILLS, and/or EXPERIENCE:

• Bachelor’s degree in social work, psychology, counseling or related field. 
• Must have experience working within the mental health field or medical fields. 
• A minimum of 2 years’ experience working with complex clients is also needed.  
• Medical case management experience a plus.
