Care Navigation Manager

Area Substance Abuse Council (ASAC) is seeking a full-time Care Navigation Manager to ensure quality patient care and linkage to services such as childcare, transportation, housing, employment, education, and basic needs to assist patients in overcoming barriers to their long term recovery. This role is responsible for direct oversight of all grant enrollment and budget monitoring, working with the Care Navigation team, ASAC team members of all disciplines and community partners to provide various services to patients and their families. 

This role starts at $53,580 per year.

as the Care Navigation Manager you will:

• Participate in hiring, selection, onboarding, training, performance management and support of Care Navigators and maintain quality performance management documentation for personnel management.
• Assist Director in formulation and organization to the overall approach of the delivery of Care Navigation services, ensuring effective implementation of policies and procedures. Evaluate ongoing needs and formulate recommendations for change. 
• Utilize data to ensure accurate grant and resource documentation and compliance.
• Complete patient file reviews to ensure quality adherence to grant, CARF and other compliance standards.
• Develop and maintain strong working relationships with community providers in all counties to facilitate navigation of resources in all counties.
• Coordinate community partner meetings and participate in collaborative action projects that improve the lives of the clients served.
• Assist Director in managing staff schedules, addressing staff needs, and stepping in to fill roles during absences or open positions to maintain consistent patient care, advocacy, and support. 

Qualified candidates will have a minimum of a Bachelor's Degree in a human services field, MA is preferred.  2 years of experience in the human services field is required. This position requires a valid driver's license and the ability to pass an extensive background check

Salary and Benefits

ASAC offers competitive compensation, this role starts at $53,580 per year with additional compensation for candidates with extensive experience.
