CHASI serves the most vulnerable individuals, families, and communities with critical services and programs. The Care Navigator is a key member of Community Health Action of Staten Island (CHASI)’s Connections to Care program at Next Step. The Connections to Care program provides comprehensive assessment, linkage, and navigation services to support the long-term substance use disorder recovery and overall well-being of individuals returning to the community from criminal justice settings.
