About Senior Doc
Founded in 2014 by Dr. Geiss, Senior Doc is a fast-growing medical group focused on delivering personalized care to seniors across multiple states. We specialize in dementia care and work closely with healthcare providers to ensure safe, effective treatment plans through our pharmacy-integrated model. Our mission is simple: improve quality of life for seniors and bring peace of mind to their families.
We’re looking for a compassionate, resourceful Care Navigator to join our team. This is position has some in person time and some in clinic work, face-to-face patient contact. You’ll play a critical role in guiding patients and families through their care journey, providing support, resources, and connection every step of the way.
Manage and maintain an assigned patient caseload
Serve as the main point of contact for patients and their families
Offer emotional support, education, and guidance over the phone
Help families access essential resources like transportation, financial aid, and local community services
Coordinate care plans in partnership with healthcare providers
Schedule appointments, follow up on referrals, and ensure timely access to care
Monitor patient progress and adjust care plans as needed
Communicate with empathy, especially during high-stress situations
De-escalate emotionally intense calls with patience and professionalism
Keep accurate, up-to-date records of interactions and care plans
Provide ongoing education and empowerment to patients, caregivers, and families
Assist with training and mentoring new staff as needed
Participate in quality improvement initiatives
Perform other duties as assigned
Excellent phone etiquette and clear communication
Strong organizational skills and the ability to manage a caseload
Compassionate, calm, and supportive during emotional conversations
Experience working with seniors or dementia care is highly preferred
Confident in handling sensitive situations and de-escalation
Team player who’s proactive and solutions-focused
Comfortable using digital tools and documentation systems
Bilingual is a plus, but not required
Must be at least 18 years old
High school diploma or GED required
Medical Assistant diploma preferred
Prior experience in patient care, case management, or senior care is a plus
