Care Partner CNA / HHA – Residential Assisted Living
Position Type: Full-Time, Part-Time, or Live-In | W-2 Employment
Location: Five residential assisted living homes in Plano and Dallas, TX
Compensation: Competitive hourly rate or daily live-in rate based on experience
Reports to: On-Site Registered Nurse (RN)
Position Overview
The Care Partner / CNA / HHA is a front-line care professional who delivers high-quality, compassionate, and resident-centered support to individuals in our boutique assisted living homes. Working under the direction of the Registered Nurse (RN) and house leadership, the Care Partner ensures that every resident receives assistance with daily living, companionship, safety, and wellness support in a dignified and nurturing environment.
Care Partner play a vital role in promoting health, comfort, and independence while strictly adhering to all care protocols and regulatory requirements. You will also assist in the execution of care plans, participate in training programs, and maintain accurate documentation — all while creating meaningful relationships with residents and families.
Core Responsibilities
Direct Resident Care
- Assist with Activities of Daily Living (ADLs) including bathing, grooming, toileting, dressing, feeding, transferring, and ambulation
- Support Instrumental ADLs (IADLs) such as light meal preparation, housekeeping, and transportation assistance
- Assist with medication administration and observe for side effects or missed doses, reporting issues to the RN
- Assist with physical therapy exercises or care tasks as assigned by the RN
- Monitor and report changes in condition, behavior, or appetite immediately to the RN
- Respond to resident call systems and emergencies with urgency and professionalism
- Provide support and companionship including conversation, reading, games, and calming interaction
Compliance & Documentation
- Follow and document care in accordance with each resident’s Level of Care (LOC) and care plan
- Complete shift notes and incident reporting accurately using care software (e.g., ClearCare or equivalent)
- Participate in audits or compliance checklists prepared by the RN or management team
- Properly handle and dispose of incontinence products in compliance with OSHA and infection control standards
Household Support
- Maintain a clean, safe, and organized living space for residents
- Assist with laundry, linens, and housekeeping in resident rooms and common areas
- Prepare and serve meals/snacks, respecting dietary preferences and restrictions
- Escort residents to dining areas, activities, or appointments when needed
- Assist in cleaning dining areas and resetting for meals
Training, Delegation, and Collaboration
- Complete initial and ongoing caregiver training provided by the RN or management team
- Accept delegated tasks from the RN, such as wound dressing changes (if within scope), dietary observations, or enhanced care protocols
- Participate in resident assessments or care plan updates as requested
- Attend team huddles, compliance meetings, or family discussions when required
- Work cooperatively with other caregivers, management, and healthcare partners
Required Qualifications
- High school diploma or GED (required)
- At least 1–3 years of caregiving experience in a home, facility, or long-term care setting (preferred)
- CNA, HHA, or PCA certification (preferred but not required)
- CPR certification (preferred or willingness to complete upon hire)
- Must be a U.S. citizen or have valid immigration status and work authorization
- Must pass background check, drug screen, and provide employment references
- Strong verbal communication and documentation skills
- Physical ability to lift, transfer, and assist residents safely throughout shift
- Ability to follow verbal/written directions and function in a team-based environment
Additional Expectations
- Maintain resident confidentiality, dignity, and rights at all times
- Wear proper uniform and identification badge during shift
- Must be alert and physically present throughout assigned schedule
- Capable of handling emotionally sensitive situations with tact and empathy
- Participate in emergency protocols, including fire drills, elopement prevention, and incident response
