What You Own

  Lead in-home consultations that uncover true care needs (not just stated preferences)

  Design care plans that are safe, realistic, and sustainable

  
  


• 
  

  Guide families through difficult decisions with clarity and confidence

  Serve as the single point of accountability for the client experience

  
  


• 
  

  Navigate conflict objectively (family vs. care pro vs. reality)

  
  


• 
  

  Continuously adjust care plans as needs evolve

Minimum Requirements

• 
  

  3–5+ years in care coordination, case management, or client-facing healthcare role

  Experience guiding families or clients through complex decisions

  Proven ability to manage multiple stakeholders and competing perspectives

  
  


• 
  

  Strong communication, documentation, and follow-through discipline

  
  

Compensation

• 
  

  $55,000 – $65,000 base 

  
  


• 
  

  Additional growth path tied to performance and scope ownership
