Care Team Assistant - Job Overview

Schedule: 8:30AM-5:30PM

Key Responsibilities

  • Revolutionize patient communication by addressing calls via protocol-based triage and ensuring timely follow-up.
  • Pioneer pre-visit planning processes to optimize the availability of critical patient information.
  • Facilitate game-changing care coordination by managing care transitions and supporting access to ancillary services.
  • Innovate medication management by assisting with medication refills and prior authorizations.
  • Transform referral processes by collaborating with referral coordination staff to ensure complete and efficient documentation.
  • Champion a culture of respect, teamwork, and safety across all interactions with patients, families, and team members.
  • Continuously improve safety and operational standards by adhering to Legacy Community Health's protocols and training.

Minimum Qualifications

  • High-school level education or equivalent; Medical Assistant Certification or equivalent is pivotal.
  • At least 2 years of experience in a dynamic ambulatory clinical setting preferred.
  • Fluency in Spanish is a significant advantage, contingent on service location.
  • Demonstrated mastery of electronic health record systems (EHRS) is essential.
  • Possess game-changing communication and customer service skills.
  • Proven ability to thrive in a high-paced, multi-tasking environment with exceptional organizational skills.
  • Strong typing and computer literacy to support data-driven decision-making.
  • Adept at managing confidential and sensitive information with the utmost discretion.
