This position is salary-exempt and participates in the community’s manager on-call rotation to help support staffing needs and urgent care issues after hours.
Position Summary
The Care Team Coordinator works closely with the Health & Wellness Director and other department leaders to support the daily care operations of the community. This role focuses primarily on supervising caregivers and medication assistants, coordinating staffing schedules, assisting with hiring and onboarding, and ensuring care staff follow regulatory and community standards.
This position is ideal for an experienced caregiver, lead caregiver, or home care supervisor who is ready to step into a leadership role.
Primary Responsibilities
Hire, train, mentor, and support caregivers and medication assistants
Create and manage caregiver schedules to ensure appropriate staffing levels
Respond to caregiver call-offs and coordinate shift coverage when needed
Monitor caregiver performance and provide coaching and accountability
Ensure staff follow assisted living regulations and community policies
Assist the Health & Wellness Director with care coordination and operational oversight
Support onboarding and training of new care staff
Minimum Qualifications
At least 2 years experience working as a caregiver in assisted living, memory care, home care, or a similar healthcare environment
Leadership experience such as lead caregiver, shift lead, supervisor, or mentoring role preferred
Scheduling or staffing coordination experience preferred
High School Diploma or equivalent
Arizona Assisted Living Caregiver Certification preferred (equivalent CNA or caregiver certification from another state will be considered)
Arizona Fingerprint Clearance Card, or ability to obtain one with a clean criminal record
Current CPR/First Aid certification, or ability to obtain upon hire
Current TB clearance, or ability to obtain prior to starting
