Work hours\:  Full time; Monday - Friday; 8\:30 – 5\:00p

Here’s What You Need

·       H.S. Diploma or Equivalent required

·       Three years’ relevant experience, preferably in a healthcare or service environment required

·       Patient Care/ Customer Service / Healthcare experience strongly preferred

 What You Will Do

·       Responsible for coordinating the clerical and administrative processes for entity care transitions management

·       Responsible for managing deparmental mail box, phone lines and/or fax

·       Supports transition planning activities as needed under the direction of care transition managers

·       Calls vendors, physician offices, clinics, etc

·       Faxes (electronic and traditional) documents as needed

·       Makes copies or gathers other data as needed 

·       Collect and enter data as needed

·       Completes other tasks and projects assigned
