Haven Home Health & Hospice is hiring a Care Transitions Navigator to build referral relationships and support patients across hospice, home health, and home care.

Ideal for candidates with experience in hospice, home health, admissions, care coordination, or healthcare marketing who enjoy relationship-building and community outreach.

Responsibilities

• Manage referral relationships with hospitals, SNFs, assisted living, and physician offices

• Educate partners on hospice, home health, and home care services

• Conduct Quality of Life consultations with patients and families

• Support appropriate, timely referrals

• Track outreach activity and referral trends

• Represent Haven in the community

Qualifications

• 1+ year experience in hospice, home health, home care, or related healthcare setting

• Experience in admissions, outreach, marketing, or care coordination preferred

• Strong understanding of hospice philosophy and interdisciplinary care

• Excellent communication and organizational skills

• Self-directed with ability to manage a territory

• Valid driver’s license and ability to travel locally
