Case Management Assistant

Job Description

Performs diversified clerical duties supporting department case management activities. Coordinates patient placement with post-acute care facilities and/or services.

Job Responsibility

• Supports case managers and social workers on assigned units with administrative aspects of transition planning activities. 
• Gathers and processes required documentation to facilitate placement.
• Communicates with post-acute care facilities to initiate patient transfers.
• Obtains appropriate insurance authorization for post-acute care services.
• Arranges for patient transportation upon hospital discharge, as required. 
• May attend interdisciplinary patient rounds. 
• Performs related duties as required.

Job Qualification

• High School Diploma or equivalent required.
• 1-3 years of relevant experience, required.
