Case Management Assistant

$23 Hourly rate

Per-diem Position: 40 hours/week

AM Shift (9:00AM - 5:30PM, Monday-Friday)

Founded in 1871 as the Hebrew Home for Aged and Disabled and previously referred to as the “Jewish Home”, the San Francisco Campus for Jewish Living (SFCJL) has a long and storied history of providing high-quality care to the community’s most vulnerable older adults. Guided by the values and principles of the Jewish tradition, our mission is to enrich the lives of older adults from all faiths and backgrounds.

Situated in the city’s Excelsior neighborhood on a state-of-the-art, nine-acre campus, SFCJL includes the 191-unit Frank Residences assisted living and memory care (open since 2020), and the 375-bed Jewish Home and Rehabilitation Center (JHRC), which has the distinction of being the largest, private, nonprofit distinct-part skilled nursing facility in the state of California. Additionally, our campus includes a 12 bed, short-term acute psychiatric unit, unique for its specialization in mental healthcare for older adults. All combined, SFCJL serves a vibrant and diverse population of more than 2,500 patients and residents annually.

Job Summary:
We are seeking a detail-oriented Case Management Assistant to join our team. The Case Management Assistant will support case managers in various administrative tasks related to the coordination of services for clients. The ideal candidate will possess strong organizational skills and the ability to maintain accurate records.

What you’ll do as a Case Management Assistant:
1. Assist case managers in maintaining client files and documentation.
2. Coordinate appointments and follow-up meetings for clients.
3. Conduct research and gather information to support case managers in developing client service plans.
4. Communicate with clients, service providers, and other stakeholders to facilitate the delivery of services.
5. Help track client progress and outcomes to ensure quality services are provided.

Requirements:
1. High school diploma or equivalent required; associate degree in social work or related field preferred.
2. Proven experience in an administrative support role, preferably in a social services or healthcare setting.
3. Strong communication and interpersonal skills.
4. Ability to prioritize tasks and manage time effectively.
5. Proficiency in MS Office and data management systems.
6. Understanding of client confidentiality and privacy regulations.
