We are seeking a full time Case Management Nurse (RN) working 5 days/week 8 hours plus call, who assumes clinical management of patients by working with health care providers, patients and families to develop cost effective plans of care. Assists with comprehensive care planning, implementation of care plans, communication and evaluation of progress toward goals. Assessment of patients and planning for discharge needs upon discharge, which may include skilled care, acute rehab, home health, durable medical equipment, etc. Ensures appropriate level of care on admission and uses InterQual guidelines or provides clinical information to insurance companies.

Requirements

• Registered Nurse or Licensed Practical Nurse with valid license from SC State Board of Nursing. Valid BLS certification
• Minimum 1 year Skilled Nursing/Acute Care experience
• Minimum 1 year experience case management/utilization review
• Preferred experience with the application of InterQual criteria
• Preferred experience with insurance authorizations and notifications
