Case Management Registered Nurse (RN) - Travel Position
Location: Mission, TX
Job Type: Travel Contract
Duration: 13 weeks
Shift: 5x8s
Pay: $1949/week
Job Description:
We are seeking a detail-oriented and compassionate Case Management Registered Nurse (RN) to coordinate and oversee patient care plans, ensuring safe, efficient, and cost-effective healthcare services. The Case Management RN will collaborate with providers, patients, and families to support transitions of care, manage resources, and promote positive health outcomes. The ideal candidate is highly organized, an excellent communicator, and skilled in-patient advocacy.
Key Responsibilities:
Assess patients’ medical, psychosocial, and financial needs to develop individualized care plans.
Coordinate care and services across multiple disciplines and healthcare settings.
Monitor patient progress and adjust care plans as needed to ensure quality outcomes.
Educate patients and families on treatment options, resources, and discharge planning.
Collaborate with physicians, social workers, and insurance providers to optimize care delivery.
Document assessments, interventions, and care coordination activities accurately.
Qualifications:
Active RN license in TX
Associate’s or Bachelor’s Degree in Nursing
Case Management certification (CCM, ACM-RN)
BLS certification, COVID vaccine, TB test
Minimum 1–2 years of clinical nursing experience
Prior case management, discharge planning, or care coordination experience
