POSITION SUMMARY:

This is a full time hourly position involving coordination, supervision, and administration of program services for adults with developmental disabilities.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Reviews and determines consumer eligibility for services.
• Coordinates Representative Payee services
• Coordinates medical, dental and other wellbeing appointments 
• Provides transportation for appointments if necessary
• Problem solving with individuals to ensure daily life quality 
• Supporting daily living needs
• Identify and acquire services that individuals qualify for
• Complete assessments in Electronic Information System (EIS) - Adult
• Record service notes in EIS - Adult
• Coordinate meetings with providers of services
• Travel to individual’s home, program or in the community 

NON-ESSENTIAL DUTIES AND RESPONSIBILITIES:

1. Performs other duties and tasks as assigned.

While performing the duties of this job, the employee will work in an office and a variety of community settings. Exposure to clients who may exhibit verbal and physically aggressive behavior. Risk to exposure to blood-borne pathogens. Noise level can vary from quiet to noisy. Travel within Aroostook County is required and on occasion, other parts of the state.

While performing the duties of the job, the employee is regularly required to walk and stand; use hands to finger, handle or feel objects, tools or controls; and talk and hear. The employee is occasionally required to sit, taste or smell, climb or balance, and stoop, kneel, crouch or crawl. The employee must regularly lift and/or move up to 10 pounds, frequently lift and/or move 25 to 50 pounds and occasionally lift and/or move up to 100 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and the ability to adjust focus. The employee will also need the strength and balance required to perform the activities associated with range of motion activities that are required as part of some individuals’ programs.

QUALIFICATIONS NEEDED FOR POSITION:

Experience and Skill Requirements: The following experience and skills are considered essential:

•  Ability and willingness to manage emotional or physical emergencies that can arise.
•  Ability to work on a team, sharing responsibilities with co-workers, accepting direction from supervisors, and contributing ideas.
•  Ability to safely accompany individuals into the community.
•  Ability to pass all required trainings, as specified by the program.

Education Requirements: The following education requirements are considered essential:

• Bachelor’s Degree from an accredited four (4) year institution of higher learning with a specialization in psychology, mental health and human services, behavioral health, behavioral sciences, social work, human development, special education, counseling, rehabilitation, sociology, nursing or closely related field, or Master’s Degree in social work, education, psychology, counseling, nursing or closely related field from an accredited graduate school.
• Valid State of Maine drivers license.
• Enterprise Information System training - Adult
• Action Note training
• All Case Manager trainings required by DHHS.
