Summary/Objective Responsibilities include reviewing emails, checking phone messages, following up on referrals, reviewing documentation, ensuring proper paperwork and consents filled out and signed, working well with other case managers, good communication skills, working with a team.

Essential Functions

Maintain confidentiality about patient information

Ability to interact with patients, co-workers, medical practitioners, and superiors in a professional manner

Proficiency in reading medical records/charts

Works well with a variety of age groups

Requirements

Required Education and Experience

Current California RN license or current California LVN license

Preferred Education and Experience

Use of the Epic EHR system
